AGREEMENT OF RELEASE AND WAIVER OF LIABILITY and TERMS and CONDITIONS 

I hereby agree to the following:

• That I am participating in Fitness and Yoga Classes and training, Health Programs, or workshops offered by JoStone Movement, LLC during which I will receive information and instruction about health, fitness, and yoga.  I recognize that fitness and yoga requires physical exertion, which may be strenuous and may cause physical injury, and I am fully aware of the risks and hazards involved.
• I understand that it is my responsibility to consult with a physician prior to and regarding my participation in the Fitness and Yoga Classes and training, Health programs, or workshops. I represent and warrant that I am physically fit and I have no medical condition, which would prevent my full participation in the Fitness and Yoga classes and training, Health Programs, and workshops.
• In consideration of being permitted to participate in the Fitness and Yoga Classes and training, Health Programs, and Workshops, I agree to assume full responsibility for any risks, injuries or damages, known or un-kown, which I might incur as result of participating in the program.
• In further consideration of being permitted to participate in the Yoga and Fitness Classes and training, Health Programs and workshops, I knowingly, voluntarily and expressly waive any claim I may have against JoStone Movement, LLC for an injury or death caused by their negligence or other acts.
• I agree to pay the amount due at the start of any Yoga and Fitness classes and training, Health Programs and workshops.  I understand and agree with the payment terms in conditions outlined as follows:
• Payment is due at the start of any session, class, or workshop with JoStone Movement, LLC. There are no refunds but exchanges are permitted. 
• I agree not to download and replicate any of this information on the course site for distribution or revenue.  It is the property of JoStone Movement, LLC.
• I agree not to use any of the information learned on this site to teach others on how to be a barre instructor. The barre certification course, online manual, and barre classes are property of JoStone Movement, LLC. It cannot be replicated and used for any barre certification program.

I have read the above release and waiver of liability and fully understand its contents. I voluntarily agree to the terms and conditions stated above.
